The aim of this assessment will allow the student to demonstrate an understanding and safe management of the acutely unwell patient to prevent deterioration and the promotion of recovery. There will be two different scenarios – you will only need to answer one of these.

The purpose of this assignment is to assess the students’ knowledge and critical understanding of anatomy, physiology and pathophysiology applied to the acutely ill adult. Their ability to identify appropriate means of observing and monitoring patients and their ability to critically reflect on the communication skills required for autonomous practice will also be examined and meet learning outcomes.


You are to choose one of the following scenarios for your 3000 word (+/- 10%) essay-based scenario:

Question 1

Fabio is a 72-year-old man who has been taking regular Ibuprofen tablets for his chronic back pain. Fabio has been experiencing abdominal discomfort for the past 24 hours but has continued taking his ibuprofen to ease the pain. This morning Fabio experienced three episodes of vomiting fresh blood. Fabio is brought into the Medical Assessment Unit by Ambulance with a provisional diagnosis of a gastro intestinal bleeding. You go to see Fabio and he appears pale and looks drained and lethargic. He complains of epigastric pain and feeling dizzy. When you check his vital signs, you will find the following parameters:

  • Respiratory rate: 26 breaths per minute
  • Oxygen Saturations: 90% on room air
  • Pulse: 124 beats per minute
  • Blood pressure: 90/ 60 mmHg
  • Capillary refill time: 4 seconds
  • Temperature: 35.8 degrees centigrade

OR Question 2

Mandeep is a 64-year-old woman who has a respiratory tract infection and was commenced on oral antibiotics four days ago. She calls her GP today as she feels her condition is worse. The GP is very concerned about her breathless condition and refers her to the medical assessment unit.

On arrival she feels very unwell, unable to speak in full sentences, looks flushed and is shivering. Mandeep is diagnosed with community acquired pneumonia. Her vital signs are as follows:

  • Respiratory rate 27 breaths per minute
  • Oxygen saturations 89% on room air
  • Pulse 120 beats per minute
  • Blood pressure: 87/40 mmHg
  • Capillary Refill Time: 5 seconds
  • Temperature: 39 degrees centigrade

Critically discuss, with reference to the relevant pathophysiology, the nursing assessment, priorities of care and nursing management of this acutely unwell patient for the first 24 hours following admission. Your answer should also demonstrate your underpinning knowledge of relevant pharmacology, psychosocial aspects as well as any potential complications
